Demineralization of the enamel forms a hole the tooth, exposing the dentin. This is the cause of toothache. A nasty toothache is a bittersweet problem, it’s really going to hurt, but it lets you know that you have a cavity, if it’s severe it’s likely the acid has exposed the inner pulp as well.
Prevention is key for most oral problems. Tooth decay is not an exception, try to eat as little sugary and starchy foods as possible, since this is what your bacteria feeds on most.
